The Claim Form for Old Cal-Shake is available below as a downloadable Adobe® Acrobat® PDF document. In order to be eligible for payment, you must fill out this claim form and return it to the Old Cal-Shake Settlement Administrator.

Old Cal-Shake Claim Form

After reviewing your claim and determining its eligibility, you will be sent a letter from the Administrator verifying the status of your claim. If more information is needed, you will be contacted by the Administrator’s office via telephone or email.
