The Claim Form for New Cal-Shake is available below as a downloadable Adobe® Acrobat® PDF document. In order to be eligible for payment, you must fill out this claim form and return it to the New Cal-Shake Settlement Administrator. Upon verification of eligibility, you will be contacted by the office of the Chief Roofing Inspector to schedule an inspection on your roof.

Once your claim is complete, it will be forwarded to the Chief Roofing Inspector within 30-60 days. If we need more information, we will contact you by telephone or email. We will always identify ourselves as "New Cal-Shake Settlement Fund."

The Chief Roofing Inspector's office will contact you to schedule an inspection to verify that Cal-Shake was or is installed, and to measure the roof.

There is always a surge in claims filed within the first few weeks of a settlement. Because each home must be inspected, it may take several months to complete the process. We will handle your claim as quickly as possible in the order received.
